How to become a Sheboygan Performing Arts Member
The Sheboygan Performing Arts welcomes new applicants to become members. Membership is limited to organizations both non-profit and profit, that produce or present performing arts in Sheboygan County. Annual dues of $100.00 are required. The money supports our joint advertising and promotional campaigns. The fiscal year starts July 1st and ends June 30th. Organizations shall send at least one representative to monthly meetings, the 3rd Wedneday, of each month, at 4:00 p.m. Each organization may present one vote for each motion made at meetings.
Follow these steps:
1. Print this page.
2. Fill out the form completely.
3. Write a check for $100 payable to The Sheboygan County Chamber of Commerce.
4. Mail the form and check to:

5. You will be notified within 90 days of the status of your application.
